About CareOS

Built to make care management work visible.

CareOS was built for independent physician practices doing real care coordination, chronic care management, remote monitoring, and between-visit follow-up — the work that too often gets scattered across systems, notes, time logs, and documentation.

Why CareOS Exists

Why We Built This

Independent physician practices across the United States are doing real work: care coordination, between-visit management, chronic disease monitoring, follow-up, documentation. Not because physicians aren’t trying — because the workflow is fragmented. CareOS was built to close that gap.

01

Care gets scattered

The follow-up that disappears into notes. Time logs, care plans, and program activity end up spread across separate systems.

02

Documentation loses context

The coordinator trying to keep everything current, with context living in different places and no single place to prepare it for review.

03

Between-visit work goes unseen

The physician trying to understand what changed. The practice trying to prove what happened. The work is real — it just isn’t visible.

That is the gap CareOS was built to close.

Founder Story

From Coordinator to Founder

In 2021, Ihsan ul Haq walked into his first job in US healthcare as a care coordinator in Lahore, Pakistan.

He had no connections in the US market. No healthcare technology background. No guarantee it would work.

What he did have was a willingness to learn the system from the inside — how CCM programs actually run, what care coordinators do between patient visits, where documentation falls apart, and why practices doing meaningful clinical work still struggle to make that work visible.

CareOS did not start as a software idea. It started inside the work.

Healthcare + operations foundation

  • Pharm-D — a foundation in medical language, chronic disease context, and the realities of patient care.
  • MBA — the business and operations lens: practice growth, team workflows, revenue capture, and the economic pressure independent practices face.

Over five years, he helped scale care-management operations across more than 10,000 enrolled US patients running CCM, RPM, PCM, and BHI programs. He managed teams, built workflows, and learned where documentation, time tracking, and revenue capture break down.

He also worked as a marketing and operations consultant for US medical offices and medical spas — learning how small independent practices think about growth, technology, and the operational gaps that hold them back.

CareOS came from all of that. Not from a whiteboard. Not from a pitch deck. From five years of watching the gap between care that happens and care that gets documented, reviewed, and understood.

Leadership

Leadership shaped inside care operations

Before founding CareOS, Ihsan led and supported care-management teams working across CCM, RPM, PCM, BHI, and related practice operations.

Led care-management teams

Progressed from team lead into department and operations leadership, guiding care-management teams day to day.

Mentored operators

Former colleagues describe his leadership as clear, ethical, and supportive — focused on helping people grow.

Improved workflows

Built systems and workflows that helped teams do better, more consistent work.

Leadership proof, summarized from colleague feedback.

The problem was not effort. It was visibility.

What We Believe

Care that cannot be seen becomes invisible care.

Physician practices should be able to see what happened for every patient in their care-management program — what was reviewed, what changed, what follow-up was needed, and what documentation was prepared for human review. CareOS exists to make that work visible, organized, and easier to review.

Boundaries

What CareOS Is Not

Not an EHR replacement

CareOS works alongside the systems practices already use.

Not a billing company

It does not submit claims, guarantee reimbursement, or replace billing review.

Not a clinician replacement

Clinical decisions, billing review, and patient-facing actions remain in human hands.

Not for hospital systems first

It is built around independent physician practices and care teams managing longitudinal care programs.

CareOS does not submit claims, guarantee reimbursement, or replace billing review.

What it is

What CareOS Is

One workspace for the work care teams do between visits — the follow-up, the documentation, the coordination that used to live in separate places.

Visibility, without the guesswork

CareOS connects patient progress, care-plan activity, and documentation context into one clear picture — so the coordinator, the physician, and the practice can all see what actually happened. Explore the platform.

People stay in charge

Documentation is prepared for human review — clinical and billing decisions stay with people, not automation. Patient Progress Intelligence shows what changed and what follow-up each patient needs.

Patient Progress Intelligence — for practices that refuse to let care become invisible.

CareOS is operated by Prima Solutions LLC, a Wyoming limited liability company established in 2024.
